ABSTRACT

La ptosis palpebral es una de las patologías más frecuentes en la consulta de oftalmología, tanto en urgencias como en el ámbito ambulatorio. El trauma del párpado superior puede provocar ptosis o retracción o una combinación de ambos. En este tipo de ptosis palpebral, su resolución mediante tratamiento quirúrgico, existiendo múltiples alternativas de procedimientos que se decidirá de acuerdo con la causas que las originan, así como la severidad de la ptosis. En este trabajo es a propósito de un caso clínico en el cual nos encontramos una paciente con ptosis palpebral traumática recidivante, utilizándose suspensión frontal con aponeurosis de músculo temporal, realizándose una variación de la técnica de Crawford disminuyendo la probabilidad de nueva recidiva


Palpebral ptosis is one of the most frequent pathologies in the ophthalmology consultation, both in the emergency room and in the outpatient setting Trauma to the upper eyelid can cause ptosis or retraction or a combination of both. In this type of eyelid ptosis its resolution by surgical treatment, there are multiple alternatives of procedures that will be decided according to the causes that originate them, as well as the severity of the ptosis, in this work is about a clinical case in which we find a patient with Recurrent Traumatic Palpebral Ptosis, using frontal suspension with aponeurosis of temporal muscle, performing a variation of the Crawford Technique decreasing the probability of recurrence.

OBJECTIVE@#To summarize the etiology mechanism and treatment of iatrogenic blepharoptosis after double eyelid surgery in Asia.@*METHODS@#To extensively review the literature related to iatrogenic blepharoptosis after double eyelid surgery, and to summarize and analyze the related anatomical mechanism, existing treatment options, and indications.@*RESULTS@#Iatrogenic blepharoptosis is a relatively common complication after double eyelid surgery, sometimes it is combined with other eyelid deformities such as sunken upper eyelid and wide double eyelid, which makes it difficult to repair. The etiology is mainly caused by improper adhesion of tissues and scars, improper removal of upper eyelid tissue, and injury of a link of levator muscle power system. Whether blepharoptosis occurs after double eyelid surgery by incision or suture, it should be repaired by incision. The principles of repair include surgical loosening of tissue adhesion, anatomical reduction, and repair of damaged tissues. The key is to use surrounding tissues or transplanted fat to prevent adhesion.@*CONCLUSION@#When repairing iatrogenic blepharoptosis clinically, appropriate surgical methods should be selected based on the causes and severity of the blepharoptosis, combined with treatment principles, in order to achieve better repair results.

ABSTRACT

INTRODUCCIÓN. La ptosis palpebral se define como el descenso o disfunción del párpado con respecto a su posición considerada como normal, pudiendo causar alteraciones del campo visual. Puede dividirse en lesiones congénitas o adquiridas, por el grado de disfunción entre otros. Blefaroplastia, es el término para la corrección de la misma. Existen varias técnicas correctivas quirúrgicas y no quirúrgicas. CASOS. Se presenta una serie de 5 casos tratados mediante abordaje quirúrgico anterior; detallando la técnica quirúrgica utilizada, con corrección de las estructuras hipertrofiadas, sección, suspensión, sutura del elevador palpebral o su encortamiento; suspensión de índole estática con el uso de fascia del paciente y finalizando el procedimiento con la confección del surco palpebral superior. RESULTADOS. Los 5 pacientes fueron tratados con técnicas quirúrgicas individualizadas obteniéndose buenos resultados estéticos y funcionales, sin complicaciones durante o después del procedimiento. DISCUSIÓN. La técnica de blefaroplastia quirúrgica dependerá del grado de disfunción; se describen tres principales: Fasanella Servat, para ptosis leve y útil para ptosis congénita leve o síndrome de Horner; Reinserción de la aponeurosis del músculo elevador, técnica que respeta la anatomía y permite regular diferentes grados de ptosis; y Suspensión del párpado al músculo frontal pudiendo ser definitiva o reversible. La edad de intervención para ptosis congénita antes del año de edad es urgente, si tapa la pupila para así evitar ambliopía y tortícolis compensatoria, en el resto de casos se sugiere realizarla a partir de los 5 años de edad. CONCLUSIONES. El conocimiento adecuado de la anatomía palpebral, etiología de la dermatocalasia y blefaroptosis, permite realizar una correcta cirugía reparadora individualizada para cada paciente. Las técnicas presentadas en este trabajo son reproducibles.


INTRODUCTION. Palpebral ptosis is defined as the descent or dysfunction of the eyelid with respect to its position considered as normal, which can cause alterations in the visual field. It can be divided into congenital or acquired lesions, according to the degree of dysfunction, among others. Blepharoplasty is the term for its correction. There are several surgical and non-surgical corrective techniques. CASES. We present a series of 5 cases treated by anterior surgical approach; detailing the surgical technique used, with correction of the hypertrophied structures, section, suspension, suture of the palpebral elevator or its shortening; static suspension with the use of the patient's fascia and ending the procedure with the confection of the superior palpebral sulcus. RESULTS. All 5 patients were treated with individualized surgical techniques obtaining good aesthetic and functional results, without complications during or after the procedure. DISCUSSION. The surgical blepharoplasty technique will depend on the degree of dysfunction; three main techniques are described: Fasanella Servat, for mild ptosis and useful for mild congenital ptosis or Horner syndrome; Reinsertion of the levator muscle aponeurosis, a technique that respects the anatomy and allows regulating different degrees of ptosis; and Eyelid suspension to the frontalis muscle, which can be definitive or reversible. The age of intervention for congenital ptosis before one year of age is urgent, if it covers the pupil in order to avoid amblyopia and compensatory torticollis, in the rest of cases it is suggested to perform it from 5 years of age. CONCLUSIONS. The adequate knowledge of the palpebral anatomy, etiology of dermatochalasis and blepharoptosis, allows a correct individualized reparative surgery for each patient. The techniques presented in this work are reproducible.

ABSTRACT

RESUMO Os aneurismas intracranianos são dilatações em segmentos arteriais que irrigam o sistema nervoso central. Acometem 2% da população e as alterações oftalmológicas podem ser as primeiras manifestações do quadro. O objetivo deste relato foi descrever um caso de aneurisma de artéria carótida interna que cursou com restrição da movimentação ocular, alteração do reflexo fotomotor, ptose palpebral, dor facial e cervical. O diagnóstico foi confirmado pela identificação do aneurisma por meio do exame de angiografia cerebral. Foi realizado teste de oclusão por balão, cujo resultado positivo possibilitou a oclusão total da artéria carótida interna por meio de ligadura cirúrgica, procedimento este realizado com sucesso.


ABSTRACT Intracranial aneurysms are dilations in segments of the arteries that irrigate the central nervous system. They affect 2% of the population and the ophthalmologic disorders may be the first evidence in the clinical examination. The aim of the report is to describe a case of an internal carotid artery aneurysm that showed restrictions of ocular movements, change of pupillary light reflex, palpebral ptosis, facial, and cervical pain. This diagnosis was confirmed by the identification of the aneurysm through angiography. A balloon occlusion test was performed, and its positive result made a complete occlusion of the Internal Carotid Artery possible through surgery ligation, procedure that was successful.

ABSTRACT

ABSTRACT Objective: To describe the upper and lower blepharoplasty technique associated with canthopexy with double pre septal orbicularis muscle elevation with a single suture in order to evaluate the efficacy of the technique. Methods: This is a retrospective study in which the medical records of 5,882 patients who underwent this technique between January 1999 and July 2015 were evaluated. The frequency and main causes of clinical complications and surgical reoperation were analyzed. Results: The incidence of complications found was 12.7% (n=750), being 0.8% (n=47) due to persistent chemosis, 3% (n=176) due to poor lower eyelid positioning, 4.17% (n=245) due to mild eyelid retraction and 4.8% (n=282) because of mild to moderate chemosis. Conclusion: The technique appears to be effective as it is simple and practical, and capable of resulting in positive functional and aesthetic outcomes with low rates of complications.


RESUMO Objetivo: Descrever a técnica de blefaroplastia superior e inferior associada à cantopexia associada à dupla elevação do músculo orbicular pré-septal em uma única sutura e avaliar sua eficácia. Métodos: Trata-se de estudo retrospectivo, no qual foram avaliados 5.882 prontuários de pacientes submetidos à blefaroplastia superior e inferior com a utilização da cantopexia, entre janeiro de 1999 e julho de 2015. Taxas e principais causas de complicações clínicas e reintervenção cirúrgica foram analisadas. Resultados: A incidência de complicações encontradas foi de 12,7% (n=750), sendo 0,8% (n=47) de caso de quemose persistente, 3% (n=176) de mau posicionamento palpebral inferior (ectrópio), 4,17% (n=245) de leve retração pálpebra e 4,8% (n=282) de quemose leve a moderada. Conclusão: A técnica mostra-se eficaz por ser simples e prática, podendo ter resultados funcionais e estéticos positivos e com baixo índice de complicações.

Objetivo: Describir los resultados funcionales de la blefaroplastia superior. Métodos: Se realizó un estudio descriptivo longitudinal prospectivo de 99 pacientes (198 párpados) con diagnóstico de dermatochalasis de párpado superior, algunos asociados a ptosis palpebral y de la ceja, en el Instituto Cubano de Oftalmología "Ramón Pando Ferrer", en el período comprendido de febrero del año 2019 a enero de 2020. Resultados: El 85,8 por ciento fueron del sexo femenino y el 43,5 por ciento eran mayores de 62 años. Predominó la dermatochalasis del párpado superior y la ptosis palpebral en el 68,4 por ciento, así como la dermatochalasis y la ptosis palpebral severa, las cuales representaron el 47,5 y el 50 por ciento respectivamente. En el grupo de 56 a 61 años sobresalió la dermatochalasis del párpado superior y la ptosis de la ceja, con el 61 por ciento. En el 98 por ciento de los casos intervenidos se obtuvo la corrección total de la dermatochalasis del párpado superior, la corrección de la ptosis en el 86 por ciento y de la ceja en el 88 por ciento. No se presentaron complicaciones en el 94 por ciento de los párpados intervenidos. La complicación más frecuente fue el sangramiento, con el 2,5 por ciento. Conclusiones: El abordaje de la ptosis palpebral y de las cejas transblefaroplastia superior logra en la mayoría de los pacientes la corrección total de estas afecciones y de la dermatochalasis. Este procedimiento constituye una herramienta importante para el rejuvenecimiento facial(AU)


Objective: Describe the functional results of upper blepharoplasty. Methods: A prospective longitudinal descriptive study was conducted of 99 patients (198 eyelids) diagnosed with upper eyelid dermatochalasis, in some cases associated to eyelid and eyebrow ptosis. The study was carried out at Ramón Pando Ferrer Cuban Institute of Ophthalmology from February 2019 to January 2020. Results: Of the total patients studied, 85.8 percent were female and 43.5 percent were aged over 62 years. Upper eyelid dermatochalasis and palpebral ptosis prevailed with 68.4 percent, whereas severe dermatochalasis and palpebral ptosis represented 47.5 percent and 50 percent, respectively. A predominance was found of upper eyelid dermatochalasis and eyebrow ptosis in the 56-61 age group (61 percent). Total correction of upper eyelid dermatochalasis was achieved in 98 percent of the cases intervened, eyelid ptosis correction in 86 percent and eyebrow ptosis correction in 88 percent. No complications appeared in 94 percent of the eyelids intervened. The most common complication was bleeding with 2.5 percent. Conclusions: In most cases, management of palpebral ptosis and upper eyebrow transblepharoplasty achieves total correction of these disorders and dermatochalasis. The procedure is an important tool for facial rejuvenation(AU)

RESUMO O presente trabalho objetivou relatar um caso de tarsal buckling associado a prolapso conjuntival e à inversão de pálpebra superior como complicação da correção cirúrgica de ptose residual. Paciente do sexo feminino, 15 anos, portadora de ptose palpebral residual unilateral em olho direito, secundária à correção parcial da ptose palpebral congênita operada na infância. A segunda abordagem cirúrgica foi realizada com ressecção da aponeurose do músculo levantador da pálpebra superior, que evoluiu com inversão conjuntival da pálpebra superior. A fragilidade estrutural do tarso é a principal hipótese para justificar o tarsal buckling subsequente à cirurgia. Houve resolução completa do tarsal buckling, porém houve também persistência da ptose palpebral. O tarsal buckling é, portanto, uma complicação cirúrgica incomum, que pode ocorrer na correção da ptose palpebral, em que há rotação posterior da metade superior do tarso, fazendo com que este se curve verticalmente sobre seu eixo, favorecendo o prolapso conjuntival. A suspeição diagnóstica e a reabordagem cirúrgica precoce favorecem a resolução da condição e previnem possíveis complicações visuais.


ABSTRACT This paper aimed to report a case of tarsal buckling associated with conjunctival prolapse and upper eyelid inversion as a complication of surgical correction of residual ptosis. A 15-year-old female patient with unilateral residual eyelid ptosis in the right eye, secondary to partial correction of congenital blepharoptosis operated in childhood. The second surgery was performed with resection of the upper eyelid levator muscle aponeurosis, which progressed to conjunctival inversion of the upper eyelid. The structural fragility of the tarsus is the main hypothesis to justify tarsal buckling after surgery. There was complete resolution of tarsal buckling, but persistence of blepharoptosis. Tarsal buckling is an infrequent surgical complication that can occur in correction of blepharoptosis, when there is posterior rotation of the upper half of the tarsus, causing it to curve vertically on its axis and favoring conjunctival prolapse. Establishing diagnosis and early reoperation favor resolution of the condition and avoid possible visual complications.

INTRODUCCIÓN. La parálisis facial refractaria produce ectropión paralítico secundario, que predispone a la queratopatía por exposición y otras complicaciones oculares, que deben ser manejadas con cirugía. OBJETIVO. Describir el manejo quirúrgico oftalmoló-gico en parálisis facial refractaria mediante tira tarsal y suspensión del pliegue nasolabial. MATERIALES Y MÉTODOS. Estudio observacional, retrospectivo. Población y muestra conocida de 8 Historias Clínicas, en el Hospital de Especialidades Carlos Andrade Marín, período enero 2016 a diciembre 2018. Criterios de inclusión: registros de parálisis facial y ectropión paralítico. Los datos fueron tomados del sistema AS400, y se analizaron en el programa estadístico International Business Machines Statistical Package for the Social Sciences, Versión 25.0. RESULTADOS. La etiología tumoral fue 62,5% (5; 8), A los 6 me-ses postquirúrgicos se observó resolución de: lagoftalmos, lagrimeo, ardor ocular y quera-titis en el 87,5% (7; 8); el ectropión se resolvió en todos los casos y se obtuvo una mejoría en la ptosis. La agudeza visual mejoró en el 75,0% (6; 8). DISCUSIÓN: La literatura evi-denció que las técnicas quirúrgicas si bien no abordan el aspecto oftalmológico y estético a la vez, aún es incierto su manejo de manera conjunta dado que ha sido poco descrita pero ha adquirido importancia por los resultados en la Unidad de Oftalmología del hospital.CONCLUSIÓN. La descripción del manejo quirúrgico oftalmológico en parálisis facial re-fractaria mediante la técnica de tira tarsal y suspensión del pliegue nasolabial fue asertiva como experiencia local.


INTRODUCTION. Refractory facial paralysis produces secondary paralytic ectropion, which predisposes to exposure keratopathy and other ocular complications, which must be managed with surgery. OBJECTIVE. Describe the ophthalmic surgical management of refractory facial paralysis using tarsal strip and suspension of the nasolabial fold. MATE-RIALS AND METHODS. Observational, retrospective study. Population and known sam-ple of 8 Clinical Histories, at the Carlos Andrade Marín Specialty Hospital, period from january 2016 to december 2018. Inclusion criteria: records of facial paralysis and paralytic ectropion. The data were taken from the AS400 system, and analyzed in the statistical pro-gram International Business Machines Statistical Package for the Social Sciences, Version 25.0. RESULTS. The tumor aetiology was 62,5% (5; 8). At 6 months after surgery, resolu-tion of: lagophthalmos, lacrimation, ocular burning and keratitis was observed in 87,5% (7; 8); ectropion resolved in all cases and ptosis improved. Visual acuity improved in 75,0% (6; 8). DISCUSSION: The literature showed that the surgical techniques, although they do not address the ophthalmological and aesthetic aspects at the same time, their joint mana-gement is still uncertain since it has been little described but has acquired importance due to the results in the Ophthalmology Unit of the hospital. CONCLUSION. The description of ophthalmic surgical management in refractory facial paralysis using the tarsal strip techni-que and suspension of the nasolabial fold was assertive as a local experience.

ABSTRACT

A síndrome de Miller Fisher é uma desmielinização dos nervos cranianos e periféricos, gerando graves consequências para o paciente, como, por exemplo, redução ou ausência dos reflexos, paralisia do III, IV e VI nervos cranianos e ataxia. Este relato descreveu o caso de uma mulher de 51 anos, natural e procedente de Penápolis (SP), admitida em um hospital de Araçatuba (SP) com quadro de arreflexia, ataxia e oftalmoplegia. No contexto clínico, foi suspeitada a hipótese de síndrome de Miller Fisher e, assim, começou o processo de investigação, com base nos critérios diagnósticos. O caso foi diagnosticado como síndrome de Miller Fisher, e o tratamento teve início.


Miller Fisher Syndrome is a demyelinating disease affecting cranial and peripheral nerves, leading to severe problems to the patient, such as reduced or absent reflexes, III, IV and VI cranial nerves palsy, and ataxia. This report describes the case of a 51-year-old woman from the city of Penápolis, in the state of São Paulo, who was admitted to the hospital in the city of Araçatuba, in the same state, with ataxia, areflexia and ophthalmoplegia. In the clinical context, the suspicion of Miller Fisher Syndrome was raised, and then investigation ensued for the disease, based on the diagnostic criteria. After evaluation, Miller Fisher Syndrome was confirmed and treatment was started.

ABSTRACT

La miastenia gravis es una enfermedad neuromuscular crónica debida a deficiencia de transmisión nerviosa en la unión neuromuscular, de origen generalmente autoinmune en el adulto, que se caracteriza por grados variables de debilidad de los músculos esqueléticos del cuerpo, que aumenta durante los períodos de actividad y disminuye después de períodos de descanso. Sin embargo en la infancia cobran especial relevancia los síndromes miasténicos congénitos, que encuentran su origen en mutaciones de genes que codifican proteínas que juegan papeles clave en el mantenimiento de la transmisión neuromuscular, teniendo edad de inicio, distribución de debilidad y respuesta a tratamiento variables. Se presentan tres casos con el objetivo de describir el comportamiento clínico de la enfermedad y la utilidad de estudios complementarios ya que es de suma importancia su precoz identificación y tratamiento. Myasthenia gravis is a chronic neuromuscular disease due to deficiency of nerve transmission in the neuromuscular junction, usually of an autoimmune origin in the adult, which is characterized by varying degrees of weakness of the skeletal muscles of the body, which increases during periods of activity and decreases after periods of rest. In childhood, however, congenital myasthenic syndromes, which find their origin in mutations of genes that encode proteins that play key roles in maintaining neuromuscular transmission, which may have a varying age of onset, distribution of weakness and response to treatment, are particularly relevant. Three cases are presented with the aim of describing the clinical presentation and course of the disease and the usefulness of complementary studies, since its early diagnosis and treatment is of paramount importance.Keywords: Myasthenia gravis, repetitive stimulation test, palpebral ptosis, neuromuscular junction, cranial pairs.

Resumen El Síndrome de Horner (SH) es una condición provocada por un grupo heterogéneo de patologías. Estas tienen en común el daño de la vía simpática. La localización más común de la lesión es a nivel preganglionar, por trauma o cirugías en región cervical, torácica o ápex pulmonar. El diagnóstico es primordialmente clínico mediante la identificación de la triada de miosis, ptosis y anhidrosis. Se confirma con las pruebas para cocaína e hidroximetanfetamina. El tratamiento se lleva a cabo primero con agentes farmacológicos. Los que tienen disminución del campo visual por la ptosis o por razones cosméticas se llevan a tratamiento quirúrgico. En ese caso los abordajes operatorios recomendados para ptosis leve son el procedimiento de Fasanella-Servat, el avance de aponeurosis del elevador y en casos severos el cabestrillo frontal. El objetivo es hacer una revisión de los algoritmos diagnósticos y terapéuticos del SH para lograr un abordaje sistemático debido a las múltiples etiologías que posee.


Abstract Horner Syndrome is a condition caused by a heterogeneous group of pathologies. These have in common the damage of the sympathetic pathway. The most common location of the lesion is at the preganglional level, due to trauma or surgeries in the cervical, thoracic or pulmonary apex region. The diagnosis is primarily clinical by identifying the triad of myosis, ptosis and anhidrosis. It is confirmed with the cocaine and hydroxymethamphetamine tests. Treatment is first carried out with pharmacological agents. Those with diminished visual field due to ptosis or cosmetic reasons are undergoing surgical treatment. In this case, the recommended operative approaches for mild ptosis are the Fasanella-Servat procedure, the elevator aponeurosis advance and, in severe cases, the frontalis sling. The objective is to review the diagnostic and therapeutic algorithms of SH in order to achieve a systematic approach due to the multiple aetiologies it possesses.

ABSTRACT Purpose: To review the outcomes of frontalis suspension surgeries with the use of polytetrafluoroethylene in patients with blepharoptosis. Methods: A retrospective observational study analyzed the outcomes of frontalis suspension surgeries performed in a single institution from 2003 to 2018. All procedures were performed with closed incision and single pentagon techniques. Outcomes were classified as satisfactory or unsatisfactory, with satisfactory defined as a margin reflex distance of >3 mm and <1 mm between eyelids and unsatisfactory as hypocorrection, surgical complications, and asymmetry. Results: We included a total of 76 eyelids from 52 patients in our study. Within a mean postoperative follow-up of 16.8 ± 18.5 months (range, 3-95), 59 (77.6%) eyelids had a satisfactory outcome, and 17 (22.4%) were unsatisfactory (8 cases of asymmetry, 3 granulomas, 3 suture extrusions, 2 abscesses, and 1 case of cellulitis). Nine eyelids from the unsatisfactory group required reoperation. Among the patients with a follow-up of ³12 months (38 surgeries), lasting results were observed in most eyelids, except for 2 late-onset suture extrusions. Conclusion: The use of polytetrafluoroethylene in frontalis suspension surgery was shown to be predictable, safe, and lasting. Our findings support previous studies that have shown adequate functional results and low complication rates.


RESUMO Objetivo: Revisar os resultados de cirurgias de sus pensão ao músculo frontal com o uso de fio de politetrafluoretileno em pacientes com blefaroptose. Métodos: Em um estudo observacional retrospectivo, foram analisados os resultados das cirurgias de músculo frontal de uma instituição, realizadas entre 2003 e 2018. Todos os procedimentos foram realizados com incisão fechada e técnica de pentágono. Os desfechos foram classificados como satisfatórios ou insatisfatórios com definição satisfatória definida como distância margem-reflexo >3mm e <1mm entre as pálpebras e insatisfatória como hipocorreção, complicações cirúrgicas e assimetria. Resultados: Incluímos um total de 76 pálpebras de 52 pacientes em nosso estudo. Com um tempo médio de seguimento pós-operatório de 16,8 ± 18,5 meses (intervalo 3-95), 59 (77,6%) pálpebras apresentaram desfecho satisfatório e 17 (22,4%) insatisfatórios (8 casos de assimetria, 3 granulomas, 3 extrusões de sutura, 2 abscessos e 1 caso de celulite). Nove pálpebras do grupo insatisfatório necessitaram de reoperação. Entre os pacientes com seguimento ³12 meses (38 cirurgias), resultados duradouros foram observados na maioria dos casos, exceto por 2 extrusões de sutura de surgimento tardio. Conclusão: O uso de politetrafluoretileno na cirurgia de músculo frontal mostrou ser previsível, seguro e duradouro, Nossos achados corroboram com estudos prévios que mostraram resultados funcionais adequados e baixos índices de complicação.

ABSTRACT

Resumo Fogo em campo cirúrgico durante cirurgia palpebral é uma complicação intra-operatória que é dramática tanto para o paciente quanto para a equipe médica. Relatamos um caso de acidente cirúrgico durante cirurgia palpebral onde o paciente sofreu queimadura de supercílio. Houve interação entre o oxigênio usado para sedação (máscara aberta) e uma fonte de ignição representada pelo cautério monopolar. Embora o paciente tenha apresentado boa evolução clínica com recuperação total da lesão cutânea, este caso é um alerta para se evitar tais tipo de ocorrência. Ressaltamos neste trabalho quais as condições implicadas e o modo de prevenção.


Abstract Fire in the surgical field during eyelid surgery is an intra-operative complication that is dramatic for both the patient and the medical staff. It's being reported a case of surgical accident during eyelid surgery where the patient suffered a brow burn. There was interaction between the oxygen used for sedation (open mask) and a source of ignition represented by monopolar cautery. Although the patient presented good clinical evolution with complete recovery of the cutaneous lesion, this case is an alert to avoid such type of occurrence. This work highlights the conditions involved and the way of prevention.

Introducción: las malposiciones palpebrales (ptosis, dermatochalasis superior y ectropión) son de los principales motivos de consulta en el área de cirugía plástica ocular. Aún no hay información concluyente en la literatura sobre los cambios corneales topográficos que se generan con las malposiciones palpebrales y si hay cambios en la topografía corneal posterior a la corrección quirúrgica de estas. Objetivo: determinar los cambios corneales topográficos de los pacientes con malposiciones palpebrales sometidos a corrección quirúrgica. Diseño del estudio: estudio observacional prospectivo longitudinal. Método: pacientes seleccionados por conveniencia a quienes se les realizó corrección quirúrgica de afecciones palpebrales (ptosis, ectropión y dermatochalasis superior) en el Hospital Militar Central de Bogotá entre abril y septiembre de 2019. Se describieron los datos de la topografía corneal previa a la corrección y al mes y tres meses después de esta y la agudeza visual mejor corregida preoperatoria y en la última evaluación postoperatoria realizada. Resultados: se realizó corrección quirúrgica de malposiciones palpebrales a 106 ojos de 54 pacientes. El cilindro medio tuvo un cambio de 0,13 D y el desplazamiento de su eje de 1, 49º. El cambio global en la queratometría media fue de 0,01 D y el grosor corneal central disminuyó 1,5 mcs. En cuanto la agudeza visual mejor corregida hubo un aumento de 0,0415 por escala LogMAR. Conclusión: la cirugía de las malposiciones palpebrales genera cambios topográficos corneales, donde la corrección de ptosis es la que más cambios ocasiona, con persistencia de los cambios a los 3 meses postoperatorio.


Background: palpebral malpositions (ptosis, upper dermatochalasis and ectropion) are the main reasons for consultation in the area of eye plastic surgery. There is still no conclusive information in the literature on the topographic corneal changes that are generated with palpebral malpositions and if there are changes in the corneal topography after surgical correction. Objective: to determine the topographic corneal changes of patients with palpebral malpositions submitted to surgical correction. Study design: longitudinal prospective observational study. Method: patients selected for convenience who underwent surgical correction of palpebral conditions (ptosis, ectropion and upper dermatochalasis) at the Central Militar Hospital of Bogotá between April and September 2019. Corneal topography data prior to correction were described and one month and three months after this and the visual acuity better corrected preoperatively and in the last postoperative evaluation performed. Results: surgical correction of palpebral malpositions was performed in 106 eyes of 54 patients. The middle cylinder had a change of 0,13 D and the displacement of the cylinder axis of 1,49 °. The average global change in mean keratometry was 0,01 D and the central corneal thickness decreased 1,5 mcs. With the best corrected visual acuity there was an increase of 0,0415 per LogMAR scale. Conclusion: palpebral malpositions surgery generates corneal topographic changes, where the correction of ptosis is the one that causes the most changes, with persistence of the changes at 3 months postoperatively.

ABSTRACT

A boy, aged 2 years and 4 months, had a sudden onset of blepharoptosis of the right eyelid, accompanied by the mouth deviated to the right side, drinking cough, nystagmus, and developmental regression. Cranial MRI showed softening lesions formed after infarction of the right dorsolateral medulla oblongata, while head CT angiography showed no imaging of the proximal part of the V4 segment of the right vertebral artery. The child was diagnosed with dorsolateral medulla oblongata syndrome and was treated with gamma globulin to regulate immune function, with mannitol to reduce neuronal edema, with low-molecular-weight heparin sodium to improve local hypercoagulation of occluded blood vessels, with hyperbaric oxygen to improve local ischemia and hypoxia and promote the recovery of brain function, and with neuromuscular electrical stimulation to promote the recovery of neuromuscular function. Before discharge, only mild right ataxia and Horner syndrome remained. This article reports the first case of infantile dorsolateral medulla oblongata syndrome and provides experience for the diagnosis and treatment of the disease.

RESUMEN Objetivo: Evaluar los resultados quirúrgicos de la técnica de elevación de la ceja transblefaroplastia y las complicaciones presentadas. Métodos: Se realizó un estudio descriptivo, retrospectivo, de corte longitudinal, de 12 pacientes (24 párpados) con ptosis de la ceja asociada a dermatochalasis del párpado superior desde mayo a septiembre del año 2018, en la consulta de Cirugía Plástica Ocular del Instituto Cubano de Oftalmología "Ramón Pando Ferrer". Fueron excluidos los que ya tenían una cirugía anterior del tercio facial superior. Resultados: El 83,3 por ciento de los pacientes fueron del sexo femenino. En cuanto a los grupos de edades resultó más frecuente el grupo de mayores de 62 años de edad. Hubo un predominio de la raza blanca, para un 58,3 por ciento. En el 60 por ciento de los casos del sexo femenino (6 pacientes), la dermatochalasis del párpado superior, asociada a la ptosis de la ceja, fue moderada. La ptosis de la ceja resultó corregida en 18 párpados, para el 75 por ciento. Se presentó el 25 por ciento de complicaciones (3 pacientes). Conclusiones: La técnica de elevación de la ceja transblefaroplastia logra la corrección de la ceja en el mayor número de pacientes con un bajo porcentaje de complicaciones, y constituye una buena opción quirúrgica para este tipo de afección palpebral(AU)


ABSTRACT Objective: To assess the surgical outcomes of the eyebrow lift technique of transblepharoplasty and the complications presented. Methods: A descriptive, retrospective, longitudinal study was carried out with 12 patients (24 eyelids) with eyebrow ptosis associated with upper eyelid dermatochalasis, from May to September 2018, in the Ocular Plastic Surgery office of Ramón Pando Ferrer Cuban Institute of Ophthalmology. Those who already had a previous surgery of the upper facial third were excluded. Results: 83.3 percent of the patients were female. Regarding the age groups, the group of people over 62 years of age was more frequent. There was a predominance of the white race, accounting for 58.3 percent. In 60 percent of female cases (6 patients), dermatochalasis of the upper eyelid, associated with eyebrow ptosis, was moderate. Eyebrow ptosis was corrected in 18 eyelids, for 75 percent. 25 percent of complications were presented (3 patients). Conclusions: The eyebrow lift technique of transblepharoplasty achieves eyebrow correction in the greatest number of patients with a low percentage of complications, and constitutes a good surgical option for this type of eyelid condition(AU)

RESUMEN La deficiencia monocular de la elevación se define como la limitación de la elevación del ojo afectado desde cualquier posición de la mirada, con ducciones normales en todas las demás posiciones. La pseudoptosis se puede presentar en posición primaria de la mirada; sin embargo, el 25 por ciento de los casos puede mostrar una ptosis verdadera. Se presenta un paciente masculino de 11 años de edad con antecedentes de estrabismo hacia adentro desde que nació, quien llevó tratamiento con oclusiones y cristales desde el primer año de edad. Al examen oftalmológico presentaba una agudeza visual mejor corregida de 1,0/0,4; a 6 metros en el ojo derecho 30 ∆ b externa ~18 ∆ b superior y en el ojo izquierdo 30 ∆ b externa ~18 ∆ b inferior; a 33 centímetros en el ojo derecho 30 ∆ b externa ~25 ∆ b superior y en el ojo izquierdo 30 ∆ b externa ~25 ∆ b inferior y limitación de la elevación en todas las posiciones horizontales de la mirada en el ojo derecho. Se realizó la prueba de ducción forzada y se encontró una restricción del recto inferior derecho, por lo que se decidió retroinsertar este músculo y ambos rectos medios. Se indicó la corrección óptica y la rehabilitación con oclusiones que mejoró la visión a 1,0/0,7. La cirugía correctiva logró la ortotropía y la mejoría de la agudeza visual(AU)


ABSTRACT Monocular elevation deficiency is defined as a limitation in the elevation of the affected eye from any position of gaze with normal ductions in all other positions. Pseudoptosis may occur in the primary position of gaze, but 25 percent of the cases are true ptosis. A case is presented of a male 11-year-old patient with a history of inward strabismus since birth, treated with occlusions and lenses as of his first year of life. At ophthalmological examination, best corrected visual acuity was 1.0/0.4; at 6 meters in the right eye 30 ∆ b outer ~18 ∆ b upper, and in the left eye 30 ∆ b outer ~18 ∆ b lower; at 33 centimeters in the right eye 30 ∆ b outer ~25 ∆ b upper, and in the left eye 30 ∆ b outer ~25 ∆ b lower, and elevation limitation in all horizontal gaze positions of the right eye. The forced duction test revealed a restriction in the lower right rectus muscle. It was thus decided to insert back the right rectus muscle and both medial rectus muscles. Optical correction and rehabilitation with occlusions improved the patient's vision to 1.0/0.7. Corrective surgery achieved orthotropy and improved visual acuity(AU)

ABSTRACT

RESUMEN Objetivos: Describir algunas características clínicas de la exotropía de gran ángulo y determinar su resultado quirúrgico. Métodos: Se realizó un estudio descriptivo retrospectivo de 35 pacientes atendidos en el Hospital Oftalmológico "Amistad Cuba-Argelia", en la Wilaya de Ouargla, Argelia, en el período enero 2014 - diciembre 2017. Se analizó el comportamiento de algunas variables relacionadas con la exotropía, como fueron la edad del paciente, el sexo, el color de la piel, las alteraciones oftalmológicas asociadas, el grado de ambliopía, el defecto refractivo asociado, el ángulo de desviación preoperatorio y posoperatorio y el tipo de cirugía realizada. Resultados: Predominó el grupo etario entre 24 y 29 años de edad con el 25,7 por ciento; el 51,4 por ciento representó al sexo femenino; el color de la piel negra fue el más frecuente con 48,6 por ciento; la ptosis palpebral correspondió al 11,4 por ciento de las alteraciones oftalmológicas asociadas y al 68,6 por ciento sin alteraciones; la ambliopía moderada se registró en el 42,9 por ciento de los casos; el defecto refractivo más observado fue el astigmatismo miópico con el 37,1 por ciento; el ángulo de desviación preoperatorio más significativo se encontró en el grupo de 71-80 dioptrías con 37,2 por ciento. A los tres meses de la cirugía el 91,4 por ciento de los pacientes se encontró en ortoforia (± 8 dioptrías) y al año de la cirugía el 94,3 por ciento se mantenía en ortoforia. El retroceso de ambos músculos rectos laterales (10,0 mm) más la resección de un músculo recto medio (7 mm) se realizó en el 37,2 por ciento de los pacientes. Conclusiones: La casi totalidad de los pacientes con exotropía de gran ángulo logran el alineamiento ocular y consiguen una mejoría en la calidad visual(AU)


ABSTRACT Objectives: Describe some clinical characteristics of large-angle exotropia and determine its surgical outcome. Methods: A retrospective descriptive study was conducted of 35 patients attending Cuba-Algeria Friendship Ophthalmological Hospital in the wilayah of Ouargla, Algeria, from January 2014 to December 2017. Analysis was carried out of variables related to exotropia, such as the patients' age, sex, skin color, associated ophthalmological alterations, degree of amblyopia, associated refractive defect, pre- and post-operative angle of deviation, and type of surgery performed. Results: The 24-29 year age group prevailed with 25.7 percent; 51.4 percent of the patients were female; black skin color was the most common with 48.6 percent; eyelid ptosis represented 11.4 percent of the associated ophthalmological alterations and 68.6 percent without alteration; moderate amblyopia was present in 42.9 percent of the cases; the most frequent refractive defect was myopic astigmatism with 37.1 percent; the most significant preoperative angle of deviation was found in the 71-80 diopter group with 37.2 percent. Three months after surgery, 91.4 percent of the patients were orthophoria (± 8 diopters) and at one year 94.3 percent remained orthophoria. Bilateral lateral rectus muscle recession (10.0 mm) plus resection of a medial rectus muscle (7 mm) was achieved in 37.2 percent of the patients. Conclusions: Practically all patients achieved ocular alignment and improved their visual quality(AU)

RESUMEN Objetivo: Evaluar la eficacia de la técnica quirúrgica Supramáxima en la corrección de la ptosis palpebral superior moderada y grave. Métodos: Se realizó un ensayo clínico aleatorizado a simple ciega, en 95 pacientes (119 ojos), provenientes de la Consulta de Oftalmología del Hospital Hermanos Ameijeiras, desde febrero del año 2016 a noviembre de 2017, los cuales recibieron tratamiento quirúrgico con la técnica Supramáxima o la técnica Suspensión al frontal. La eficacia del tratamiento se evaluó en cuanto a: caída del párpado, altura del pliegue palpebral, distancia margen reflejo y satisfacción del paciente. Resultados: El promedio de edad fue de 59,35 años, con leve predominio del sexo femenino (56,8 por ciento) y de la ptosis palpebral grave (51,6 por ciento). La diabetes mellitus fue la enfermedad sistémica más frecuente (56,8 por ciento). La eficacia terapéutica de la TSM fue superior (96,7 por ciento) a la TSF (86,2 por ciento). Se constató una mayor satisfacción del paciente con la TSM (97,9 por ciento). Conclusiones: La técnica quirúrgica Supramáxima es más eficaz que la suspensión al frontal en la corrección de la ptosis palpebral superior moderada y grave(AU)


ABSTRACT Objective: Evaluate the efficacy of the Supramaximal surgical technique for correction of moderate and severe upper eyelid ptosis. Methods: A randomized single-blind trial was conducted with 95 patients (119 eyes) from the Ophthalmology Service of Hermanos Ameijeiras Hospital from February 2016 to November 2017. These patients had undergone surgery by either Supramaximal technique or frontal suspension technique. Efficacy of the treatment was evaluated in terms of eyelid ptosis, eyelid crease height, margin-reflex distance and patient satisfaction. Results: Mean age was 59.35 years, with a slight predominance of the female sex (56.8 percent) and of severe eyelid ptosis (51.6 percent). Diabetes mellitus was the most common systemic condition (56.8 percent). The therapeutic efficacy of SMT was higher (96.7 percent) than that of FST (86.2 percent). Patient satisfaction was higher with SMT (97.9 percent). Conclusions: The Supramaximal surgical technique is more effective than frontal suspension for correction of moderate and severe upper eyelid ptosis(AU)

ABSTRACT

RESUMEN Los quistes epidermoides constituyen el 1 % de los tumores intracraneales y el 7 % de los del ángulo pontocerebeloso. Los colesteatomas son lesiones benignas que se originan de restos de tejidos epitelial ectodérmicos que quedan en el sistema nervioso central, al cerrarse el tubo neural entre la tercera y quinta semana de gestación. Se trata de un paciente remitido de la Consulta de Neurooftalmología con crisis de cefalea y toma de los pares craneales III, IV, V, rama oftálmica desde hace 2 semanas. Se le realizaron estudios imagenológicos donde se constató un tumor hipodenso en región silviana frontotemporal izquierdo. Se interpretó como un quiste arcnoideo. Se le aplicó un bordaje pterional transilviano con apoyo neuroendoscópico y para sorpresa del equipo quirúrgico se abordó un tumor perlado solido identificado macroscópicamente como un colesteatoma silviano. Se resecó la totalidad del tumor cerebral.


ABSTRACT Epidermal inclusion cysts constitute 1% of the intracranial tumors and 7% of the cerebellopontine angle ones.Cholesteatoma are benign lesions originated from the remains of ectodermic epithelial tissues remaining in the central nervous system when the neural tube closes between the third and fifth week of pregnancy. The case deals with a patient remitted from the Neurophthalmologic Consultation with migraine crisis and lesion on the III, IV, V cranial nerves, ophthalmologic branch, for two weeks. Image studies were carried out, showing a hypo dense tumor in the left silvian frontotemporal region. It was taken as an arachnoid cyst. A pterional trans-silvian approach with neuroendoscopic support was applied, and the surgical team was surprised when they found a solid pearly tumor that was macroscopically identified as a silvian cholesteatoma. The cerebral tumor was totally resected.
